Step One: Gastric Sleeve
The first step of the duodenal switch is to perform sleeve gastrectomy. During a duodenal switch, however, our surgeon removes slightly less of your stomach than they would during a gastric sleeve procedure. By reducing the size of your stomach, the calories you can consume will be restricted.
Step Two: Intestinal Bypass
The second step of duodenal switch surgery is to bypass your small intestine. There are two ways to perform this part of the surgery, through the biliopancreatic diversion (the original) or the SADI-s, or loop procedure. Each version will bypass a large portion of your small intestine, restricting the number of calories your body absorbs.
SADI-S
The Modern Duodenal Switch
SADI-S, or the loop duodenal switch, is similar to the biliopancreatic diversion in that it still divides your small intestine near the duodenum and then reattaches the lower portion to the top, bypassing a large section of the intestine. However, there are two key differences in these procedures.
Intestinal Bypass
During the traditional duodenal switch, around 80-90% of your small intestine is bypassed compared to 50-60% in the SADI-S procedure.
Simplified Process
Unlike the original method that attaches your small intestine in two places, SADI-S loops your small intestine, allowing it to reconnect in one location.
Gastric Bypass vs. Duodenal Switch What Is the Difference?
Duodenal switch surgery is similar to gastric bypass, or the Roux-en-Y method, in that both reduce your stomach size and bypass your small intestine. However, gastric bypass focuses more on restriction, while duodenal switch focuses more on malabsorption. During the Roux-en-Y procedure, your stomach size is reduced by about 70-80%, and about 30% of your small intestine is bypassed. During duodenal switch surgery, your stomach size is reduced by around 60-70%, and about 75% of your small intestine is bypassed.
